OK, the web page at http://www.jedec.org/Home/MIDCODE_request.cfm implies 
you have to pay, but notice the link at the top. You don't have to pay. 
You can't redistribute the document. You can get an ID for $200. 

This whole thing is really weird and so annoying I'm tempted to set up a 
page and let people tell me vendor IDs they get from data sheets so that 
you can easily find this stuff.
